How do I register for the organ rally?

Option #1 - Click on the "Download File" link below to Print the registration form and complete the information by hand.

version of the file is made to be viewed and printed. The printed copy may be filled out by hand and mailed to my address, which is listed under the contact information. You cannot type responses into this type of file.

Click the file below to download it. This file is a Microsoft Word file and requires Microsoft Word (part of Microsoft Office) to open the file. This version of the file is formatted as a fillable form. Just type your responses into the gray boxes and then save the file. You have two choices: 1) You may print the file and mail it to my address, which is listed under the contact information
2) you may email me the form as an attachment. Feel free to choose whatever option is easiest for you.
